Some employers won’t let people return to work after injuries etc until they have a doctor say it’s okay. This would be like if you injured your shoulder not-at-work but you work as a construction worker. Construction company doesn’t want you back at work until a doctor says they’re clear.
If people can’t get in to see their doctor, they might be out of work for weeks, so every once in awhile, someone will come in asking for a ‘return to work’ note.
You’re right though, you need to double-triple verify. “RETURN” to work note, not a “GIMME THE WHOLE WEEKEND and MONDAY OFF” work note?

Blame the employers for treating their workers like children in some cases. Heaven forbid they take the day off while they get through the stomach flu or some such. It’s job security for some, to prove to that employer you were not fooling around, but were indeed sick enough you needed to see a physician.
Usually if I’ve had to be off from work due to illness, it’s been something I’d need to see a physician for. Anything else I will drag myself in and suffer through.
The other commenter was right too. When I had surgery my place of employment wouldn’t allow me to return to work without the doctors written okay. I would have gone back in sooner, but he had to physically see me and sign the slip for return.

Ahh, you must have seen my husband then. The man could come to the ED with both his legs missing and he’d want a note that says he could go back to work.
When he actually had to be admitted for heart problems, I felt bad for the nurses, cause the second they gave him some scrub pants to wear under his gown, I swore he was secretly plotting how to escape without disconnecting his heart monitor and go back to work.

I’ve nearly had to do that before; regular doc was out of town for a week, but luckily another local one was willing to fax a note without even seeing me. Failing that, it would have been either a trip to the ER for a note, or nine days of missed work for a two day bout of diarrhea. “I had the runs, Dr. D. prescribed Lomotil. Now I wish I had the runs” was enough to convince him that I was good enough to go back.
